Employees will need to disclose how many PTO hours they would like to use, if any, per pay period by filling out the attached LOA PTO Request form (Please click on the PDF below to download the form).
Please complete the LOA PTO Request form and submit it to HR@Hoag.org by Thursday (Before the pay period end date) to get paid for the following payroll date. For example, if we receive the form by Thursday, January 6, 2022, then you will receive your PTO hours on January 13, 2022, pay check day. Late submissions will be paid the following payroll week. Please refer to the Payroll calendar.
Please note - you should not request more than 30% of PTO if you are receiving State Disability Insurance (SDI) or Paid Family Leave (PFL).
For example, if you work 40 hours per week and will be receiving SDI/PFL, then you should not request more than 12 hours of PTO per week or a total of 24 hours per pay period.
If you are using PTO while on Leave, your payroll deductions for benefits premiums will continue on a bi-weekly basis.
If you choose not to use your PTO, you will still be responsible for submitting a payment to Hoag Benefits Center for any health insurance premiums you have elected through Hoag. As a reminder, if an employee does not make the required premium payments within 30 calendar days from their premium date, he/she will have their health insurance and other benefits canceled effective the first of the following month and will have the option to transition to COBRA.
