Beginning a Leave
- All requests for Leaves of Absence (LOA) must be submitted to Sedgwick. Employees requesting any leave of absence must provide at least thirty (30) days advance notice when the when the need for the leave is foreseeable. The leave can be initiated via telephonic claim submission to Sedgwick by calling (877) 221-0688 or online at timeoff.sedgwick.com/. Employees can also go online to “check” their leave status. To register as first-time user, please refer to the Employee Portal Registration Instructions page.
- It is important to notify Sedgwick of any changes related to your return-to-work status ASAP as this may affect your compensation and/or benefit deductions.
- After submitting a LOA request, an acknowledgement leave Packet will be mailed to the employee’s mailing address. The packet would include necessary forms, notices, and FMLA Rights and Responsibility notice. Completed Certification Forms must be submitted to Sedgwick to the attention of the assigned case manager via email or via fax or mail:

- Approval/Denial notifications will be sent by Sedgwick to the immediate supervisor via e-mail advising of employee’s leave and approximate duration. It is the employee’s responsibility to maintain their supervisor informed of any new leave requests, changes related to their leave status including extensions, return-to-work, etc.
Modifying a Leave
Employees requiring a leave modification (change in duration or type of leave):
- Contact Sedgwick Case Manager – (877) 221-0688 or timeoff.sedgwick.com.
- LOA modification packet will be sent to employee (if required) for completion
- Leave modification approval/denial notice will be mailed to employee
- Copy of the approval/denial notice will be sent to supervisor via e-mail
Returning to Work
- Employee will receive a Return-to-Work Form in their leave packet.
- All Return-to-Work Release Forms must be completed and returned to Sedgwick (no exceptions) at least two (2) calendar days prior to the scheduled leave end date, if out for their own serious medical condition.
- Completed Return to Work form must be faxed or mailed to Sedgwick at (888) 436-9535 or PO Box 182808 Columbus, OH 43218-2808
- Failure to provide the completed Return-to-Work Release form directly to Sedgwick at least two (2) calendar days prior to the scheduled leave end date, will result in not being able to remain at work until Sedgwick provides a date. In addition, computer/badge and other access may not be reactivated in a timely manner.
- Sedgwick will notify the immediate supervisor via email when the Return-to-Work Release is received. Unless the supervisor receives the email notification from Sedgwick, employee cannot return to work from being on LOA due to their own health condition.
- It is important to notify Sedgwick of any changes related to your return-to-work status ASAP as this may affect your compensation and/or benefit deductions.
- Sedgwick will follow up with the employee one day later to confirm he/she has in-fact returned to work.
- After hours and weekend employees cannot work without authorization from their department supervisor.
