Eligible employees have the option of using PTO during their leave of absence.
To submit a request, please complete the electronic PTO Request Form. This form is to be used by employees on leave of absence wanting to use their PTO. Please indicate how many hours of PTO you would like to use per pay period while on a leave of absence. All requests are due to Human Resources no later than 5:00pm on Thursday of the pay period's end date. PTO requests that are submitted after the deadline will be processed on the following pay period.
