To initiate a Leave of Absence, please contact Sedgwick at 877-221-0688. Employees can also go online to check their leave status. To register as first-time user, please refer to the Employee Portal Registration Instructions page.
After submitting a LOA request, an acknowledgment leave Packet will be mailed to the employee's mailing address. The packet would include necessary forms, notices, and FMLA Rights and Responsibility notice. Certification information (forms) must be submitted to Sedgwick to the attention of the assigned leave manager or via fax or mail:

Approval/Denial notifications will be sent to the immediate supervisor via email advising of employee's leave and approximate duration. It is the employee's responsibility to maintain their supervisor informed regarding their return to work status.
